Opportunity Home is committed to combating fraud, waste and program abuse and encourages all staff, program participants, and the public to report such activities

WHAT KINDS OF COMPLAINTS CAN I SUBMIT?
We take seriously all credible allegations regarding:

Housing Fraud
Violations to the lease, family obligations, regulations, or other program requirements.

Customer Service
Complaint where an issue was not addressed and resolved to the client’s satisfaction and additional information or follow up is required.

Criminal Activity
Complaint where the law is violated, such as criminal and drug activity.

Employee Complaint
A complaint regarding Opportunity Home staff where it appears a violation of expected Code of Conduct has been committed.

Alert eyes and ears of law-abiding citizens are often key to uncovering instances of wrongdoing. We welcome and encourage you to report suspected activity in one of the following ways:

To act on your allegation, we need you to provide as much identifying information as possible regarding the suspect, victim, and the details of the incident. The more you can tell us, the better chance we have of determining whether a violation has been committed.

Your information should include the following:

  • Who committed the fraud or program violation?What exactly did the suspect do?
    • Name
    • Social Security number
    • Date of birth
    • Address
    • Telephone Number
  • Where did the incident occur?
  • When did it happen? Is it still taking place?
  • How did you become aware of the incident ? Were you there?
  • Were any other persons involved?
  • Has anyone else (e.g., police, property manager, etc.) been notified?
  • Documentation supporting your claim, such as the following:
    • Police reports or police report numbers
    • Court documents
    • Written correspondence
    • Photographs
    • Video

 
If you are a program participant, you may also need to provide:

  • The last four digits of your Social Security number
  • Current phone number and address
  • The housing program you are in (e.g., Public Housing, AHP, or Non-Profit)

Opportunity Home understands that informants may believe disclosing their identities may create problems or place them in danger. We will protect the identity of complainants to the maximum extent allowed by law, and only release your identity to those officials who have a need to know. Opportunity Home employees who report allegations may also request confidentiality.

MAY I REMAIN ANONYMOUS?
Yes. However, please keep in mind that your decision for anonymity may limit our ability to conduct a complete investigation. We ask that at a minimum you leave a method of contact (telephone number or e-mail) in the event that the investigator requires more information.

DISCLOSURE POLICY
Opportunity Home reserves the right to act or not to act on any information provided unless it is required to do so by law. Furthermore, Opportunity Home cannot provide information regarding what action has been taken on any allegation reported to us. Federal regulations prohibit the disclosure of information contained in law enforcement records, even to the individual making the allegations. Unless you are contacted directly by one of our investigators, there will be no communication from our office. Under no circumstance will we provide you with the status of an action taken on the allegation. If you are a Opportunity Home employee, Opportunity Home may not take action against you solely because of your submission of an allegation to the Opportunity Home fraud hotline. Federal laws protect employees from reprisals by their employers for “blowing the whistle” on illegal activity.

PRIVACY ACT NOTICE
Information that you submit will be used to conduct investigations into fraud, waste, and abuse in connection with the programs and operations of Opportunity Home. Your submission is voluntary, and there is no penalty for failing to report a possible violation. However, you may be subject to possible criminal or civil penalties for intentionally reporting false information to Opportunity Home. You should report the truth, as you know it. We may make disclosures of information you submit to others, including other federal and state law enforcement agencies, as necessary to complete our investigation, or as required under federal law.
